PLEASE READ & AGREE TO WAIVER
ALL PARTICIPANTS AND VOLUNTEERS IN THE Irvine Family Fun Fair AND RELATED EVENTS AGREE TO ASSUME ALL RISK OF
PARTICIPATION AND VOLUNTEERING IN THE Irvine Family Fun Fair
AND OTHER THE Irvine Family Fun Fair RELATED EVENTS BY SIGNING
THIS GENERAL RELEASE AGREEMENT.
The undersigned volunteer
(Volunteer) on behalf of himself/herself and on behalf of Volunteer's
personal representatives, assigns, heirs, executors, hereby fully and
forever releases, waives, discharges and covenants not to bring any
legal action or seek to hold liable the City of Irvine, Irvine Unified
School District, Irvine Community Drug Prevention and any or all of the
agencies affiliated with the Irvine Prevention Coalition, all municipal
agencies whose property and/or personnel are used and all other
sponsoring or co-sponsoring companies or individuals related to The
Irvine Family Fun Fair and related events, and the directors,
officers, affiliates, employees, representatives or other relations to
such entities (collectively, the Releasees), from all liability to the
Volunteer and his/her personal representatives, assigns, heirs and
executors, for all loss(es) or damage(s) and any and all claims of
demands therefore, on account of injury to the Volunteer or property or
resulting in the death of the Volunteer, whether caused by the active or
passive negligence of all or any of the Releasees or otherwise, in
connection with the Volunteer's participation in The Irvine Family Fun Fair and related events.
The Volunteer represents and
warrants that he/she is in good physical condition and is able to safely
volunteer in The Irvine Family Fun Fair and/or related events.
The Volunteer is fully aware of the
risks and hazards inherent in volunteering in The Irvine Family Fun Fair and/or any related events, knowing the risks associated with
The Irvine Family Fun Fair and/or any related events. The
Volunteer hereby assumes all risks of loss(es), damage(s), or injury(ies)
that may be sustained by him/her while participating/volunteering in the
Marathon and/or any related events. The Volunteer agrees to the use of
his/her name and photograph in broadcasts, newspapers, brochures, and
other media without compensation and without notice. The Volunteer
hereby authorizes medical treatment as needed. The Volunteer warrants
that all statements made herein are true and correct and understands
that Releasees have relied on them in allowing Volunteer to participate
in The Irvine Family Fun Fair and/or related events.
NOTE: Minors must have signed permission by a Parent or Legal Guardian.
If your age on the day of the event is less than 18 year old, you must
have your parents complete the Parent Permission Form (we will send this
to you when we receive your registration). You must bring the completed form with you on the day
of the event. Thank You. |
